Obesity is one of the most pressing health concerns in Malaysia and across Asia. Malaysia has the second- highest adult obesity rate in Southeast Asia, with a prevalence of 19.7%. According to the National Health and Morbidity Survey (NHMS) 2023, 54.4% of Malaysians are either overweight or obese. This contributes to the increasing incidence of non-communicable diseases, including diabetes, heart disease, and certain types of cancers.

Dr Pok Eng Hong, Consultant General and Upper GI Surgeon at Ara Damansara Medical Centre (ADMC), explains, “Obesity is far more than a cosmetic concern. It is a chronic disease with significant implications for both physical and mental health. It is crucial to understand that this is not just about appearance but about saving lives.”

Obesity is not simply about overeating or a lack of self-discipline. It is a complex condition influenced by a combination of genetic, hormonal imbalances, environmental factors, and lifestyle habits.

Dr Pok shares, “Many patients come to me feeling as though they have failed because diets and exercise didn’t work for them. What they don’t realise is that their struggle is not just about willpower. Metabolic and hormonal factors often play a significant role, making weight loss incredibly challenging.”

The impact of excess weight extends beyond physical health, significantly affecting mental well-being.

Many individuals struggle with low self-esteem, social isolation, and depression, yet the stigma surrounding obesity often prevents them from seeking the help they need. “Many people believe they have to face this challenge alone, but obesity is a medical condition that requires medical solutions,” says Dr Pok.

A personalised approach to weight management

Effective weight management requires a holistic and personalised approach. No single solution works for everyone, and treatment plans should be tailored to each individual’s needs, goals, and health status.

Obesity is a global crisis, but it can be addressed through awareness, medical advancements, and early intervention. “Taking action today can pave the way for a healthier tomorrow,” concludes Dr Pok. “With the right medical guidance and support, you can break free from the cycle of weight gain and regain. It is never too late to take control of your health.”

For individuals who are not ready for surgery but require medical support beyond diet and exercise, obesity medications offer a valuable solution. These medications are designed to regulate hunger, feelings of fullness, and metabolism.

According to Dr Pok, “Medications like GLP-1 receptor agonists have transformed the landscape of weight management. These drugs mimic natural hormones that signal satiety, which helps individuals control portion sizes and achieve significant weight loss. When combined with lifestyle changes, they offer an effective way to break the cycle of weight gain and loss.”

Other options include gastric balloons, which are non-surgical, minimally invasive solutions for individuals who need help managing their appetite and portion sizes. The procedure involves inserting a balloon into the stomach through the mouth, which is then filled with saline to create a feeling of fullness that suppresses hunger. The balloon is typically removed after six months to one year.

According to Dr Pok, “The gastric balloon is an excellent option for those looking to lose 10–15% of their body weight while making significant lifestyle changes. It serves as a supportive bridge for patients who need extra help without committing to surgery.”

Bariatric surgery is for individuals with severe obesity (BMI over 35 or those BMI over 30 with obesity-related health issues). Bariatric surgery is often the most effective and long-lasting solution.

Procedures like gastric bypass and sleeve gastrectomy change the digestive system to help limit food intake and, in some cases, decrease nutrient absorption.

Dr Pok emphasises the transformative impact of bariatric surgery: “This isn’t just about losing weight. It is about resolving serious health conditions such as type 2 diabetes, high blood pressure and sleep apnea.

The benefits are life-changing. Patients often tell me that they feel as if they have been given a second chance at life.”

The urgency of early intervention

Delaying treatment for obesity can lead to worse health outcomes and reduced quality of life. Many individuals hesitate to seek help, often waiting for the “right time” or hoping for another diet to be effective.

Dr Pok warns against this mindset: “Obesity is a progressive condition. The longer you wait, the more complications you may face. Early intervention not only prevents further damage but also significantly improves your chances of success.”

He also addresses the emotional and mental challenges that many patients face. “Seeking help is not a sign of failure, it is a brave step towards reclaiming your health. Obesity is a medical condition, not a character flaw. With the right tools and support, it is entirely possible to make a positive change.”

Dr Pok said that medical interventions are not quick fixes; they are tools designed to help you achieve and maintain better health.
